this is not common for a 3 yr old child. it's common for new born till 1 yr age. plz consult a doctor around you for the same.
few changes can help reduce this surely :
1. regular oiling the child's hair
2. use child hair mild shampoo
3. use only Luke warm water to wash hair.
4. check haemoglibin and b12 levels which can also result in this problem.
5. fungal infection of scalp cam also cause this. which is more common in rainy season.

sometimes yes ; but do consult a doctor , my cousins Doctor had same issue for her daughter and she was consulted by homeopathic Doctor and the problem got cleared..
