How to Obtain Birth Certificate?
My husband left abroad when my daughter was just 2 weeks old, It was difficult for me to do things especially getting Birth Certificate, Insurance etc. I was trying to get information from all possible resources few were helpful but few confusing, Hope this will help people in future.
1️⃣ When you get discharged your hospital will provide you yellow paper (the colour might be different in each hospital). The paper is nothing but a copy of your baby's information like when she/he was born, parent's name, hospital name etc. The original will be sent to the local municipality office by the hospital. So make sure you keep the yellow paper safe.
2️⃣ My doctor asked me to go to the municipality office after 1 month but my neice got hers in 10 days, So it's always better to check with the doctor when we can go. Either one of the parent or anyone on your behalf can go to the municipality office to fill the form, If you are sending any of the relative then ask them to carry the parents original Aadhar card and 1 copy of each. No need of any authorization letter from the parents required.
3️⃣ The form can be filled online as well but again requires lot of information where we need to search, So I felt its better to fill the form in the office and ask the staff if we have any questions. While filling the form make sure you enter the correct name, spellings etc. Once done it can't be changed. So if your sending any of your family members better to write everything on a paper and give.
4️⃣ Once the form is filled and submitted along with Adhar Card they will provide a Receipt and date and time to collect the Birth certificate. Better to go in their stipulated time. They do not charge anything here but in few places they charge anything between Rs. 10 to Rs. 50, you can also request more than 1 copy.
5️⃣ The time they take is 1 week or less, Anyone who has the receipt can go and collect.
*️⃣ Here in Bombay they were not ready to take the middle name.. THEY SAID ONLY 2 NAMES WILL BE TAKEN, NOT SURE ABOUT OTHER CITIES.
*️⃣ The municipality office is where the hospital is located.


Do not delay much in getting your birth certificate.

Becca! Cant thank u enuf for this! Very helpful! For ini we also followed similar except no aadhar in those days :) they asked for our marriage certificate. And we took 10+ <u>copies</u>
